Extended Leave of Absence Process for Faculty Members

extended leave of absence facultyDepartments must submit the completed Extended Leave of Absence application on behalf of faculty members to University Benefits for review before the applications are sent to the Provost for approval.  Once University Benefits confirms the application is complete, the application will be sent back to the faculty member’s department.  The department must forward it to the Provost’s office for final approval.  Provost approval is only required for faculty extended leave requests.
