Assess your risk and learn more about diabetes

Did you know that one in three adults in the U.S. has pre-diabetes, and of those, more than 84% don’t know they have it?

Determine your risk factors
November is Diabetes Awareness Month, and UF-UF Health Wellness has developed a quick questionnaire to help you determine if you are at risk for prediabetes and type 2 diabetes. At the end of the assessment, you’ll be provided with a summary of your results to share with your doctor when you schedule your next visit. Additionally, you will receive a short survey in 6 months to track what changes you may have made. All of these answers will be kept confidential and secure.
