It’s tax time — be on the lookout for your W-2 and 1095-C forms
By Jan. 23, employees who consented to receive an electronic Form W-2 should be able to access it through myUFL or the ONE.UF application. Those who did not consent or who separated from UF more than eighteen months ago should receive their printed tax forms by mail soon after Jan. 30.
In addition to filing your W-2, the IRS requires reporting of health insurance coverage information for the 2025 federal income tax reporting year. While it is not necessary to include Form 1095-C with your tax return, you will need to include your insurance information.
State of Florida Plans
1095-C forms for state health plans may be obtained through your People First account at: Insurance Benefits > Health Insurance Tax Forms.
Your People First ID is available in myUFL at: Main Menu > My Self Service > Benefits > PFID & Beneficiary Info.

GatorCare and GatorGradCare
Historically, the University of Florida has mailed IRS Form 1095-C to all employees who were enrolled in GatorCare or GatorGradCare and eligible for health insurance per the Affordable Care Act (ACA). Beginning with calendar year 2025, these forms will be provided by request only. You may request a physical or electronic copy of your form by contacting UF Benefits.
Who do I call if I have questions about this Form?
If you have questions about the Form, please call your tax preparer or visit the IRS website at http://www.irs.gov/ACA.
If you have questions about the personal information on the Form (for example, your address or Social Security number is incorrect), please contact UFHR University Benefits at (352) 392-2477 or via email at benefits@ufl.edu.
What should I do if I did not receive or if I lost the Form?
State health plan participants may log in to People First, click on the “Insurance Benefits” tile, then click on “Health Insurance Tax Form.” You may also call the People First Service Center at (866) 663-4735 and request a duplicate copy.
GatorCare health plan participants should contact UFHR University Benefits at (352) 392-2477 or via email at benefits@ufl.edu to request a duplicate copy.
